Perineoplasty: What Is It?

Perineoplasty is a surgical procedure that repairs and restores the perineal area and external genital tissues, collectively known as the vaginal opening. The perineum is the region between the vagina and the anus, playing an important role in supporting the pelvic organs and pelvic floor muscles. Various factors—such as childbirth, weight loss, or trauma—may damage this area, leading to loosened or widened tissue around the vaginal opening.

The primary goals of perineoplasty are to strengthen and tighten the perineal area, reduce the size of the vaginal opening, and repair loose, scarred, or damaged tissues. Correcting these concerns helps the perineum function properly, supports pelvic floor health, and enhances overall comfort.

In addition to health benefits, perineoplasty can improve the aesthetic appearance of the perineal region, helping enhance body confidence. Patients who experience discomfort or dissatisfaction due to changes in the perineum may benefit from this procedure, which supports both physical and emotional well-being.

Reasons to Consider Perineoplasty

Many individuals undergo perineoplasty to address laxity or loosened skin around the vaginal opening, which may affect sexual satisfaction or lead to self-consciousness. In more severe cases, damaged muscles may contribute to urinary or fecal incontinence.

 

Common Reasons for Choosing Perineoplasty:

  • Reduced confidence in genital appearance

  • Decreased libido or diminished sexual desire

  • Urinary or fecal incontinence

  • Pain, irritation, or discomfort around the vaginal area

  • Loose, protruding, or excessively widened vaginal opening

  • Scarring from childbirth or episiotomy

  • Reduced sensation during intercourse

 

Causes of Perineal Damage

The leading cause of perineal damage is childbirth. During vaginal delivery, the opening of the vagina must stretch to allow the baby to pass through. Multiple vaginal deliveries increase the likelihood of loosening and widening of the vaginal opening.

Severe perineal tears during childbirth or episiotomy may result in scarring or incomplete muscle healing, which can lead to discomfort or reduced sensation in the area.

Risks of Perineoplasty

As with any surgical procedure, perineoplasty carries potential risks and complications, including:

  • Infection

  • Constipation

  • Bleeding at the surgical site

  • Inadequate tightening of the perineum or vaginal opening

  • Pain during intercourse (dyspareunia)

 

Before the Procedure

Perineoplasty is performed by a gynecologist. Patients should discuss their symptoms, concerns, and goals with their doctor to fully understand the benefits, risks, and expected recovery. It is also important to review preoperative instructions and what to expect during the healing process.

Surgical Procedure

Perineoplasty can be performed under local anesthesia or general anesthesia, depending on the patient’s condition and the complexity of the surgery. During the procedure, the surgeon makes an incision at the vaginal opening to remove excess skin or scar tissue and tightens the surrounding vaginal muscles. The incision is then closed with sutures.

The surgery typically takes about one hour. After a short recovery period in the clinic or surgical center, most patients can return home the same day without the need for an overnight hospital stay.

 

Postoperative Care

Healing takes time, and patients may experience swelling and discomfort in the first few days. Most doctors recommend cold compresses and pain medication to reduce discomfort. Warm sitz baths may help soothe irritation during early recovery.

General Postoperative Instructions:

  • Use sanitary pads if light vaginal bleeding occurs

  • Keep the surgical area clean and dry; absorbable stitches will dissolve on their own within a few weeks

  • Avoid tampons and sexual intercourse for at least 4–6 weeks

  • Avoid strenuous activities, such as heavy lifting or intensive exercise, for at least 4 weeks

  • Eat a high-fiber diet and drink plenty of water to prevent constipation; stool softeners may be helpful to avoid straining

Many doctors also recommend pelvic floor exercises (Kegel exercises) after the healing period to strengthen the muscles around the perineum. Some patients may be referred to a pelvic floor physical therapist or given home exercises to support recovery.

Expected Results

The typical recovery period is 6–8 weeks. During this time, following medical advice closely is essential. Patients should avoid strenuous exercise for at least 4 weeks and refrain from sexual intercourse for at least 6 weeks to ensure complete healing.
